Virtual care
Changes to the MSC Payment Schedule related to virtual care will take effect November 1, 2026.
During the COVID-19 pandemic, temporary changes to fee-for-service (FFS) billing policies helped support the rapid expansion of Telehealth services. The 2022 Physician Main Agreement (PMA) provided an opportunity to take a broader look at virtual care, with the Ministry of Health reviewing both existing FFS policies and the temporary measures introduced during the pandemic and bringing forward proposed revisions.
The proposal was reviewed by the Virtual Care Working Group in 2023, leading to a set of agreed-upon changes that were approved by the Tariff Committee and Doctors of BC Board of Directors. Their implementation was later stipulated in the 2025 PMA.
Beginning November 1, some of this work will take effect through amendments to sections D.1 (Telehealth) and D.3.3 (Counselling) of the General Preamble. The updates will make several temporary virtual care measures permanent and provide greater clarity around the requirements for providing and billing virtual care.

Key updates
- Expanded definition of Telehealth. Telehealth will include services provided by telephone as well as video. The General Preamble will also define synchronous services, such as telephone and video calls, and asynchronous services, such as email, text and voicemail
- Choice of care modality. Physicians, in consultation with their patients, are responsible for determining the most appropriate modality for providing care.
- Same-day virtual and in-person visits. A Telehealth visit and an in-person visit provided by the same physician, or their in-person substitute, on the same day are generally not both payable. Both may be payable if a new assessment is required because the patient’s condition has changed or the patient presents with a different condition.
- Patient eligibility. Telehealth services are available to MSP beneficiaries, as well as eligible residents of Alberta and Yukon under reciprocal billing arrangements. This is intended to support existing care arrangements for border communities, rather than general Telehealth practices serving patients in other provinces.
- Physician location. Physicians must generally be located in BC to bill MSP for Telehealth services. An exception applies when a physician is temporarily outside BC for 30 consecutive days or less. Exceptions may also be made for programs approved by the Medical Services Commission.
- Virtual counselling. Section D.3.3 (Counselling) will be updated to recognize counselling provided by telephone as an insured MSP benefit. Counselling provided by video call is already an insured MSP benefit.
Temporary Telehealth billing rule remains in place until April 2027
A temporary billing rule introduced during the COVID-19 pandemic allows physicians to provide services virtually when a Telehealth-equivalent fee is not available. Under this rule, physicians can bill the applicable in-person consultation, visit or non-procedural intervention fee, provided “telehealth” is identified on the claim.
This temporary rule will remain in place until April 1, 2027, giving MSP and Doctors of BC additional time to identify areas of the Payment Schedule where appropriate Telehealth fees are not currently available and address potential gaps before the rule ends.

See the updated General Preamble D.1 (Telehealth) and D.3.3 (Counselling) for the complete requirements.
