HDC is a physician-led data-sharing initiative that encourages self-reflection and collaboration with colleagues while facilitating quality improvement in patient care through its application, HDC Discover. The application is available at no cost to you or your practice. Coaching support is available for enrolment and quality improvement opportunities.

Practice Support Program (PSP), a Family Practice Services Committee initiative
The PSP offers tailored, hands-on services that help family physicians and teams become more efficient, reduce administrative burden, and create more time for proactive patient care and for implementing aspects of the patient medical home model.

Physician Quality Improvement (PQI), a Specialist Services Committee initiative
The PQI initiative is open to all physicians and addresses gaps in quality structures related to physician participation in QI activities and ensures that those physicians have adequate dedicated technical support.
