Reducing risks of frailty for healthy aging - resources for doctors and patients

Dr Grace Park is a firm believer in the collaborative community approach to caring for older adults, with a large focus on prevention, particularly as it applies to frailty – an approach that is also recommended by Doctors of BC as stated in their Healthy Aging and Preventing Frailty policy statement

Prevention and early intervention is key when you consider the statistics.

In almost every country in the world, the proportion of seniors 65 years and over is growing faster than any other age group. By 2031 in Canada, seniors will account for 22.8% of the population. This rapid and disproportionately aging population predisposed to frailty, is placing increasing pressure on an already over-burdened health care system (Park et al, 2014).

Raising awareness of how to prevent, mitigate, or even reverse factors associated with frailty for improved quality of life for patients, families, and caregivers has been a focus of Dr Grace Park’s work in her capacity as the Regional Medical Director of Home Health/Specialized Community Health Services for the Complex Medical/Frail at Fraser Health.

Dr Park has also been working with Shared Care’s Complex Care for Older Adults Initiative to develop resources to support physicians as they engage with patients around steps they can take to reduce risks.    

Frailty Toolkit

The Frailty Prevention Toolkit was created for doctors to print and share, and includes a brochure and poster covering five key frailty prevention strategies:
