Long-term and Palliative Care

Long-term and palliative care was introduced into the LFP Payment Model in June 2024. Payment for services in long-term and palliative care facilities is based on time and interactions, with rates based on the common workflows for long-term and palliative care from admission to end of life/discharge. There is also a premium on after-hours direct patient care time.

The Family Practice Services Committee (FPSC) developed new on-call/availability payments for long-term and in-patient care. Please visit the FPSC website for more information on the new on-call/availability payments.
